Subsequent installments are due on the same day of each consecutive month until paid in full. Treatment times direr from pmtient to patient. These payment options do not correspond to the estimated treetment ffme, but are merely provided foryour convenience. A late fee will be charged on all accounts which are ten days past due.

Dental Care: The above case fees include orthodontic treatment only. General dental care (extractions, cleanings, fillings, etc.) is not included in these fees. It is the patient's responsibility to maintain regular dental check-ups with their general dentist during orthodontic treatment.

Additional Fees: After the third occurrence, there will be a fee for excessive breakage of appliances. A charge will be incurred for returned checks. If treatment extends beyond the estimated length due to patient non-compliance there will be additional charges based on the extended length of time required. Fees are applicable to removal and/or replacement of orthodontic wires or appliances per occurrence as necessitated for outside dental or medical appointments. Office visits after completion of treatment may be subject to fees. Retainers: The orthodontic treatment fee includes one initial set of retainers. Lost or broken retainers will incur replacement charges per appliance.

Payment and Insurance: Monthly payments are due each month regardless of appointment status. Payments not received after 10 days of contracted due date are considered past due and are subject to a late fee. Patients who carry any form of insurance for orthodontics should know that all services performed in this office are charged directly to the patient and/or the responsible party. We will assist you by filing insurance claims, and any payment received from them will be credited towards your account balance. In the event your insurance company doesn't pay the estimated benefit, you will be responsible for all remaining unpaid fees including the insurance balance. Most insurance misunderstandings can be avoided by periodically contacting your insurance carrier to verify that your claims are being paid.

Transfer Pee: Should the patient need to transfer treatment (move/relocation out-of-area) to another orthodontist there may be a one- time fee to cover record duplication. Your account must be in good standing for the contract to be cancelled upon the transfer of the patient records.

Aligners: Due to the significant laboratory costs associated with aligner treatment, should the patient or responsible party discontinue aligner treatment for any reason, a discontinuation fee will be charged. A change of treatment charge may apply for major changes to the aligners during treatment (Mid-course correction). It is recommended that dental work be completed prior to Invisalign treatment. Aligner treatments include up to 2 revisions/refinements. Additional refinements incur additional costs.

Diseontinuation: Orthodontic services may be discontinued for lack of patient cooperation, poor oral hygiene or failure to adhere to the above financial agreement.

If treatment is completed before the end of the contract payment period,

scheduled payments are still due until the end of the agreed payment period.

Consent:

I certify that I have read and understand all of the above, and agree to make the payments as outlined above.

I am aware of the office's Insurance policy, have reviewed the Orthodontic Insurance Benefits Information, and understand any unpaid benefits will be my sole responsibility.

I have been made aware of and understand the pros and cons to the treatment deemed necessary by the doctor.

A holder of this medical debt contract is prohibited by Section 1785.27 of the Civil Code from furnishing any information related to this debt to a consumer credit reporting agency. In addition to any other penalties allowed by law, if a person knowingly violates that section by furnishing information regarding this debt to a consumer credit reporting agency, the debt shall be void and unenforceable.
